If you are a follow up patient and your appointment is IN CLINIC: Arrive 15 minutes prior to your appointment for your pre-assessment.

To prepare for your visit:

• Have your medications on hand.

• Have your lab tests done 1-2 weeks prior to your appointment (if applicable)

• If you require translation, ensure that you have an interpreter
