Introducing the Diabetes Triage

LMC’s well-known Diabetes Rapid Assessment Network has evolved further to better meet the needs of our physicians and patients: the LMC Diabetes Triage.

The Diabetes Triage links together 19 regional Endocrinologists, with Dietitians and Diabetes Educators, to provide rapid access, support and comprehensive education all at no cost for the patients. With the implementation of this program LMC Endocrinology Centres will play a role in increasing capacity in our health care system by providing assessment and treatment for at least 1200 newly-diagnosed individuals with diabetes each year.

How it works?

Within 1 Week:

  • Step 1: Endocrinology Assessment
  • Step 2: Immediate Dietitian / Diabetes Educator Assessment

Weeks 2 – 4:

  • Step 3: Ongoing Support through one-on-one and group sessions
  • Step 4: Immediate access into our Ministry Funded Diabetes Education Center
